Form VL5 Application to surrender an SPSV licence

Surrendering results in permanent deactivation of the vehicle licence. Once surrendered, a licence can never be restored, renewed or used ever again. You should complete this form only if you are sure you no longer wish to keep the licence. Remember that some taxi licences can be transferred to another person See Information Guide (G5) for further information on the surrender process. Note that, for security reasons, the Authority may contact you to confirm your request to surrender your SPSV licence.
