Form TP1 - Declaration - Failure to return tamper-proofs

You are obliged to return your tamper?proof licence discs whenever you apply to renew, transfer, surrender or change the vehicle on your licence1. If you do not return the tamper?proof discs, you will be required to submit documentary evidence that the tamper?proof discs in question have been satisfactorily destroyed e.g. a signed declaration that the tamper?proof discs were destroyed by you personally.
